The National Flood
Insurance Reform Act of 1994 requires federally regulated lenders to
use FEMA's Standard Flood Hazard Determination Form (SFHDF), FEMA Form
81-93, when determining whether a building or mobile home offered as
security for a loan will be located in a Special Flood Hazard Area. The
OMB conducted a periodic review of the SFHDF, which expired on October
31, 2008, as required by the Paperwork Reduction Act, and issued a
revised Form with a new control number. The revised Form, with a
December 31, 2011 expiration date, and instructions are available on
the FEMA Web site at

Note that the Form's format and content have not changed.
To allow users of the revised Form time to update their systems to
the new version, the effective date for mandatory use of the new Form
was set for June 16, 2009. SFHDFs completed on or after that date must
be completed using the new Form for compliance purposes. The current
Form may be used until then.
